To request the Kansas City, Missouri Health Department to be present at a function, meeting, presentation, celebration  or other event type, please fill out and submit the Health Department online outreach/health fair form.

To allow sufficient time to organize representation from the Department the form must be completed and submitted online at least 3-4 weeks prior to the date of the event.

Please be sure to include accurate contact information to ensure Department staff is able to contact event organizers regarding details, questions and/or arrangements.
